Liability Waiver

I, the undersigned, acknowledge and understand that my child, as well as other participants, will have the opportunity to interact with live animals during humane education program. I understand that while Longmont Humane Society staff and volunteers will make every effort to ensure the safety and well-being of all participants and animals, there are inherent risks associated with handling and being around animals, including but not limited to scratches, bites, or allergic reactions. By signing this waiver, I agree to the following:

1. Rules and Safety Guidelines: My child and all other participants under my care will follow the rules and guidelines provided by Longmont Humane Society staff and volunteers at all times. Failure to adhere to these rules may result in removal from the program..
2. Assumption of Risk: I voluntarily assume all risks associated with participation in this program, including interactions with live animals.
3. Release of Liability: I hereby release and hold harmless Longmont Humane Society, its employees, volunteers, agents, and affiliates from any and all claims, demands, actions, or causes of action arising out of or related to any injury, illness, or damage sustained during participation in the program

Photo/Video Release: I grant Longmont Humane Society permission to take photographs and videos of me/my child and other participants during the humane education program. These images may be used for promotional, educational, or fundraising purposes, including but not limited to social media, brochures, newsletters, and the organization’s website. My child’s name will not be published alongside any image without additional consent. I may opt out of the photo release by checking the opt out box below. Should you choose to opt out I/they will not partake in any group photos while onsite such as donation drop off photos.
