Thank you for your interest in our Paws and Pathways teen field trips through Longmont Humane Society. These field trips are designed for teens ages 13 to 19 and provide opportunities to explore animal welfare, veterinary medicine, animal advocacy, humane education, and careers that support animals and the community.
Because these programs take place off-site and may include animal-related activities, a parent or legal guardian must complete this waiver before any teen participant may attend.
By completing this form, I give permission for my teen to participate in the selected Paws and Pathways field trip. I understand that field trips may include transportation, walking, outdoor activities, interaction with animals or animal-related facilities, and participation in educational programs led by Longmont Humane Society staff, volunteers, and/or partner organizations.
I acknowledge and understand that my teen, as well as other participants, may have the opportunity to be around or interact with live animals during this program. I understand that while Longmont Humane Society staff and volunteers will make every effort to support the safety and well-being of all participants and animals, there are inherent risks associated with handling or being near animals, including but not limited to scratches, bites, allergic reactions, illness, injury, or other accidents.
By signing this waiver, I agree to the following:
Rules and Safety Guidelines:
My teen will follow all rules, directions, and safety guidelines provided by Longmont Humane Society staff, volunteers, and partner organizations at all times. I understand that failure to follow these expectations may result in my teen being removed from the activity or restricted from future programs or field trips.
Assumption of Risk:
I voluntarily assume all risks associated with my teen’s participation in this field trip, including transportation, walking, outdoor activities, educational activities, and interactions with live animals or animal-related facilities.
Transportation:
I understand that this field trip includes transportation to and from the field trip location. Transportation will be provided by a professional charter bus company with trained drivers. I understand that, as with any travel by vehicle, there are inherent risks associated with transportation, including but not limited to traffic conditions, accidents, delays, or other unforeseen events. I voluntarily assume these risks as part of my teen’s participation in this field trip.
Release of Liability:
I hereby release and hold harmless Longmont Humane Society, its employees, volunteers, agents, affiliates, and partner organizations from any and all claims, demands, actions, or causes of action arising out of or related to any injury, illness, damage, or loss sustained during participation in the program.
Emergency Medical Care:
In the event of an emergency, I authorize Longmont Humane Society staff to contact the emergency contacts listed on this form and to seek emergency medical care for my teen if necessary.
Limited Space:
I understand that space is limited. If my teen is no longer able to attend, I agree to notify Longmont Humane Society as soon as possible so the spot may be offered to another participant.
